Cognitive Assessment and Resources Evaluation for Lifelong Independence
What we all want is to age gracefully and with dignity. The "C.A.R.E. Lifelong" assessment is an in home or virtual assessment that focuses on cognitive and communication components of home safety and management to maximize your ability to safely stay in your own home as you age.Â
Your therapist will:
assess your current living situation, medical history, and support network
asses your level of independence with home and personal management
identify current or potential safety issues or barriers to independence
After the evaluation, you will have:
recommendations and training for adaptations and changes to your home and personal management including but not limited to:
home reorganization
automating household, financial, and/or medical management
use of smart devices
A ready to go binder of emergency documents and plans
an easily accessible list of local community resources that you may benefit from now or in the future
A sense of calm and confidence that you are ready to age with grace in the setting of your choice
**This is a one visit assessment; follow up sessions or check in sessions may be booked based on results of the evaluation and your comfort with completing recommendations independently**
